Video-assisted thoracoscopic pneumonectomy: the Edinburgh posterior approach.

چکیده
Twenty years of experience in Video-assisted thoracoscopic lung surgery (VATS), have led to the development of innovative techniques to rival open thoracotomy for major lung resections (1,2). Acceptability of the technique relates to benefits including; reduced length of hospital stay, decreased blood loss, decreased pain, improved cosmesis, earlier return to normal activities and improved tolerance of chemotherapy (3-6). Whilst the technique was initially limited to pulmonary lobectomy, centers are now routinely performing VATS pneumonectomy. Whilst it remains unclear as to whether the technique affects survival, VATS pneumonectomy provides a safe alternative to open resection, which confers many of the benefits outlined above (7,8). An additional plus, is that patients are not subject to postoperative air leaks, which are one of the major complications of VATS lobectomy, often resulting in prolonged hospital stays and increased morbidity and mortality (9,10). The technique of VATS pneumonectomy described here is that which is currently employed in the unit of cardiothoracic surgery at the Royal Infirmary of Edinburgh.

Video-assisted thoracoscopic surgery (VATS) was introduced nearly 2 decades ago and has experienced an exponential increase for lung cancer treatment. A pneumonectomy can be performed by video-assisted thoracoscopic surgery and the lung usually fits through the incision as usually used for VATS lobectomy. The most common approach for pneumonectomy is undertaken with 3 or 4 incisions, including ...
متن کاملVideo-Assisted Thoracoscopic Management of Post-Pneumonectomy Empyema
BACKGROUND Post-pneumonectomy empyema is a major therapeutic challenge in thoracic surgery. The presence or absence of a concomitant bronchopleural fistula directs treatment of this condition. When there is no bronchopleural fistula the condition is classically treated with thoracostomy drainage, irrigation and antibiotic instillation with closure. This approach is, however, associated with a s...

Ann Cardiothorac Surg 2012;1(4):533 www.annalscts.com This video demonstration represents our first attempt at video-assisted thoracoscopic extrapleural pneumonectomy (EPP) for a 51-year-old patient with right-sided malignant pleural mesothelioma following neoadjuvant chemotherapy (Video 1). While originating as an initial inspection for a possible radical decortication, it was apparent that th...
متن کاملVideo-assisted thoracoscopic lobectomy: the Edinburgh posterior approach.
Video-assisted thoracoscopic surgery (VATS) is now well established as an alternative to open thoracotomy for major pulmonary resections of bronchogenic carcinoma and benign disease (1,2). Compared to open surgery, the minimally invasive approach has a number of benefits in the immediate post-operative period that include reduced pain, better pulmonary function, shorter hospital stay, improved ...
متن کاملComparison of acute phase reaction and postoperative stress in pigs undergoing video-assisted thoracoscopic versus thoracotomy pneumonectomy
BACKGROUND Video-assisted thoracoscopic surgery (VATS) has been used for many thoracic diseases as an alternate approach to thoracotomy. The aim of this study was to compare the surgical outcome of pneumonectomy using VATS with that using thoracotomy pneumonectomy in pigs. Fourteen pigs were equally divided into two groups; one group underwent VATS and the other group underwent transthoracic pn...
